Membership Application

Thank you for your interest in the Japanese Society of Neuropathology (JSNP).

Type of Membership
	Regular Member (Dues:12,000 Yen per year): Subscription to Neuropathology and Neuropathology-Supplement. Associate Member (Dues:6,000 Yen per year) : Membership for students and Laboratory Technicians. Subscription to Neuropathology-Supplement only.
	*	If you wish to receive Neuropathology, please sign up for Regular Member.
Payment of Membership Fee
	1.	Bank transfer
	*	Make sure to complete payment of membership fee for you to be admitted as a JSNP member.
	*	Fiscal year starts on April 1st and ends on March 31st next year.
Required Documents
	1.	Application form with a letter of recommendation. (Required for all who join JNSP) * If it is difficult to get recommendation from JNSP Councilor, please attach a letter with your condition to the application form.
	2.	Students must submit a copy of student ID. Laboratory Technicians must turn in Certificate of a Laboratory Technician with a signature of a JSNP member. Download: Application form (PDF file) Certificate of a Laboratory Technician (PDF file)
